*Please remember to do your Semester Response Form at the beginning of each semester to Authorize Your Financial Aid, Waive Your Health Insurance and Authorize the Release of Information to a Parent. If you are waiving your student health insurance please don't forget to visit Gallagher and finish waiving it on their site.

All forms MUST be returned to us from your student email account, with the exception of the signature page and statement of purpose (which must be mailed).
